I've heard of similar cases before. Apparently there was once a couple trying to get pregnant, they had heard of sex, but because they couldn't get pregnant they tried some alternative openings. Anyways, after trying to enter via her urethra both partners were hospitalised. The man for an impacted penis, the woman for a split urethra.
Ouch.
